We like to keep things simple. Before your first visit, you're welcome to fill in our new patient form online at your own pace. Prefer to do it when you arrive? We have an iPad ready at the front desk. And if forms on screens aren't your thing, we're happy to hand you a good old-fashioned paper version.

Our new patient form covers your dental and general health history, but it also asks how happy you are with your smile and whether you snore. Your answers help us build a complete picture of your health and make sure we're prepared for anything you'd like to discuss.

We're always happy to help find appointment times that work for you and talk through payment plan options that suit your situation.
